He has two death certificates, one filed on 7 May 1935 in Ohio, and one filed on 6 May 1935 in Kentucky.

The Kentucky death certificate is typed. It records Henry C. Caudill, age 43 years 5 months 21 days, residence Germantown, Ohio, born 14 Oct 1891 in Kentucky, died 9:30 pm on 5 May 1935 at Fort Thomas, Campbell Co., Ky, divorced, occupation enlisted man, U.S. Army, father John T. Caudill born in Farmers, Ky, mother’s maiden name unknown, cause of death “fracture of skull, compound, comminuted, depressed, severe, involving parieto-occipital region with fracture extending into base of skull. 2. Fracture of left tibia and fibula, compound, comminuted, involviing upper third of both bonts. Accident, near Germantown, Ohio, struck by automobile while walking on German-Middletown Road”, informant John T. Caudill, Farmers, Rown Co., Ky. The medical section is signed by Wm. E. Hall, Lt. Col., M.C. USA, M.D., Station Hospital, Ft. Thomas, Ky.

The Ohio death certificate is hand-written. It reverses the residence and place of death, stating that he died in Germantown, Montgomery Co., Ohio, and that his usual residence is Fort Thomas, Ky. Most of the information is identical. The cause of death statement is much briefer, “Viewed remains 8:30 pm Compound Skull Fracture Fract both legs Accident on public highway Montgomery Co., Ohio struck by auto”, signed by W. H. Harris, Coroner Montgomery Co., Ohio.

It appears by comparing the two, especially the times on the medical statements, that he really died in Ohio, but his body was transported almost immediately to Fort Thomas for examination by the army medical officer rather than being held in Ohio. (Fort Thomas is only about 45 miles from Middletown, just across the Ohio River from Cincinnati.)

Clearly his father John T. Caudill knew Henry’s mother’s maiden name, since she was his wife, but for some reason it did not get entered.

1920 census of Campbell Co., Ky, Fort Thomas, enumerated 9 Jan, lists on p. 4A, with a long list of soldiers:
Caudill, Henry C., 28, single, soldier in U.S. Army, b. Ky Ky Ky.

I have not found him in 1930; most likely his name was misindexed.
