The informant Everett Fraley is Polly’s grandson, the son of William Fraley and her daughter Rinda Hitch, from her marriage to Calvin Hitch.

1900 census of Morgan Co., Ky, district 6, enumerated 19 June, lists on p. 16B as family #261/263:
Lewis, Polly, 48, b. May 1852, Ky Tenn Ky, widowed, day laborer; Hitch, Henry, son, 27, b. Oct 1872; Hitch, Lizzie, dau, 22, b. Apr 1878; Hitch, Edmund, son, 13, b. Mar 1887; Hitch, William, son, 10, b. Apr 1890. The Hitches all born Ky Md Ky.

Polly and Calvin’s children Ellen, Delpha and John are living in Devil’s Fork, Elliott Co., as servants in three different households. Apparently the widowed Polly was unable to support them all at home.

The surname of Polly’s second husband is from the 1900 census listing, which listed her as a widow Polly Lewis. I have attached those census notes to her rather than to either family notes, so that it will show up on all her family group sheets. The marriage was apparently brief, since in her marriage record to Nathan Cox in 1904 she gave her name as Polly Hitch. I could not find any record of a Lewis/Hitch wedding in Morgan Co., so they may have married in Elliott Co.
