His death certificate records William Fredrick Culp, age 71 years 1 month 14 days, born 16 Oct 1872 in Kentucky, died 30 Nov 1943 in Nelsonville, Athens Co., Ohio, widowed, late wife Lulu Hayden, occupation coal miner, father Fred Culp, born in Germany, mother’s maiden name Eliza Bellamy, born in Virginia, cause of death lobar pneumonia, informant Charles Culp. Charles apparently preferred listing his mother rather than William’s second wife Ida.
Census
1910 census of Athens Co., Ohio, enumerated 21 April, lists on p. 5B as as family #104/108:
Culp, William F., 37, b. Ky Ky Ky, second marriage, coal miner; Ida, wife, 41, b. Ohio Ohio Ohio, second marriage, married 0 years, mother of 1 child, 1 living; Charles, son, 15; Susan, dau, 12; Ethel, dau, 10; George, son, 5. All children born Ohio Ky Ohio. Courtwright, John, father-in-law, 83, widowed, b. Ohio Ohio Ohio.

Since William and Ida had only been married less than a year, but Ida is listed as the mother of 1 living child, either Ethel or George may be from her first marriage, and thus a stepchild of William. On the other hand, since Ida is 41, it is very possible that her living child is over 20 and not in the household, and that Ethel and George are both children of William’s first wife Lulu Hayden.
