His death certificate records Henry Jacob Ellington, age 48 years 5 months 12 days, residence 386 Benham, Ky, born 9 Feb 1888 in Morgan Co., Ky, died 21 July 1936 in Benham, Harlan Co., Ky, widowed, late wife Golden Cox Ellington, occupation coal miner, father R. B. Ellington, mother’s maiden name Rodie Maxie, both born in Morgan Co., Ky, cause of death acute alcoholism, informant Lindsay Ellington, #386 Benham, Ky, burial in Benham, Ky.

It is probable that he was really born in Feb 1889, which was the date given on the 1900 census, since his parents were married in March 1888.
