His obituary in the Ashland Independent reported:

“LOUISA — Dr. Herbert Bernal Lewis, 88, a highly respected and loved member of the Louisa community who practiced dentistry in the city for 55 years, died March 21, 2011, after a brief illness.

The son of the late Farmer and Carma Lewis, Dr. Lewis was born in Blaze on Sept. 22, 1922. He finished Breckinridge Training High School and attended both Morehead State University and the University of Kentucky before graduating from the University of Louisville School of Dentistry in 1943.

After serving in the South Pacific during World War II as a captain of the U.S. Army Medical Corps, Dr. Lewis returned to West Liberty and opened a dental practice. He moved to Louisa in 1947, where he continued to practice dentistry until retiring in 2002.

Dr. Lewis was a longtime member of the Louisa Rotary Club and Louisa City Council and served as a director of Louisa First Federal Savings and Loan. He was also an influential member of the First Baptist Church of Louisa for nearly 60 years, where he served as deacon and treasurer.

He is survived by his wife, Adrienne Moore Lewis, and predeceased by his first wife, Lovena Wells Lewis. He is also survived by a son, Richard Kyle Lewis of Louisa; a granddaughter, Kyla Click (Kelly) of Louisa; great-grandchildren Tanner Castle and Kenlie Click; and a brother, Garland Lewis of Eminence. He was also predeceased by two other brothers, Alden and Gordon Lewis.

The family thanks the staff at King’s Daughters Medical Center, Three Rivers Medical Center and J.J. Jordan Geriatric Center.

The funeral will be at 2 p.m. today, March 24 at First Baptist Church of Louisa with Pastors Ric Frazier and Charles Price officiating. Burial will be in Greenlawn Cemetery.”
