His WW I draft card, filed 12 Sep 1918, records Frank Purvis, age 40, born 24 Feb 1878, residence Crix, Rowan Co., Ky, farmer, self-employed, nearest relative Anna Purvis, wife.

His death certificate records Frank Purvis, age 72 years 2 months, born 25 Feb 1878 in Hilda, Ky, died 18 May 1950 in Morehead, Rowan Co., Ky, carpenter, married, father Harm Purvis, mother’s maiden name Rebecca Adkins, cause of death squamous cell carcinoma of neck, informant Mrs. Frank Purvis, burial in Brown Cemetery.

Death certificates of his siblings (both older and younger), as well as Sarah’s own death certifiate, are consistent that his mother’s maiden name was Sarah Lee, not Rebecca Adkins, so Frank’s widow apparently was confused about her mother-in-law’s maiden name.

Hall, mother’s maiden name Susan Roberts, cause of death gangrene of rt lower extremity due to diabetes mellitus, informant Ray Purvis, burial in Brown Cemetery.

Rowan Co. marriage book 3, p. 320, records marriage on 19 Aug 1899 of Frank Purvis, age 21, to Annie Hall, age 24, at James Tolliver’s.
Census
I cannot find either Frank or Annie in the 1900 census.

In 1910 Annie, Cecil and Otie are living with Annie’s parents; see their census notes for details. Meanwhile, Frank was living in West Virginia with Addie Cornett, listed as married 2 years, although they may not have really been married. (Frank seems to have followed the same pattern as my great-great uncle Johnny Pettit, who was keeping Ty Trent as a common-law wife in West Virginia, having two children with her while still being married to Malissie Clark in Kentucky. But Frank came back to Annie, while Johnny ended up eventually divorcing Malissie and marrying Ty.)

1920 census of Rowan Co., Ky, Morehead precinct, enumerated 5 Jan, lists on p. 2A as family #35/37:
Purvis, Frank, 42; Annie, wife, 44; Cecil, son, 19; Ota, dau, 15; Charles, son, 6; Ray, son, 4. All born Ky/Ky/Ky.

1930 census of Rowan Co., Ky, Morehead precinct 10, enumerated 7 April, lists on p. 3B as family #69:
