Rowan Co., Ky birth records list Fraklin Pervis, born 6 Jan 1878, father Hiram Pervis, mother’s maiden name Sarah R. Lee, both parents born in Rowan Co., Ky.

His WW I draft card, filed 12 Sep 1918, records Frank Purvis, age 40, born 24 Feb 1878, residence Crix, Rowan Co., Ky, farmer, self-employed, nearest relative Anna Purvis, wife.

His death certificate records Frank Purvis, age 72 years 2 months, born 25 Feb 1878 in Hilda, Ky, died 18 May 1950 in Morehead, Rowan Co., Ky, carpenter, married, father Harm Purvis, mother’s maiden name Rebecca Adkins, cause of death squamous cell carcinoma of neck, informant Mrs. Frank Purvis, burial in Brown Cemetery.

Death certificates of his siblings (both older and younger), as well as Sarah’s own death certifiate, are consistent that his mother’s maiden name was Sarah Lee, not Rebecca Adkins, so Frank’s widow apparently was confused about her mother-in-law’s maiden name.
Notes for Addie Cornett
In her father’s will dated May 1931 she is listed as Addie Cornett Cox of Matoka, W.Va.; in her mother’s obituary in Sept 1959, she is mentioned as a survivor, “Mrs. Addie Cox, Bluefield, W. Va.” (Matoaka is about 12 miles north of Bluefield; both are in Mercer Co.)
Census
In the 1930 census of Matoaka, Mercer Co., West Virginia, enumerated 11 April, she is living alone, listed as head of household, but her marital status is shown as married, not widowed or divorced:
Cox, Ada, 39, age at first marriage 22, born and both parents born in Kentucky.
Notes for Frank & Addie (Family)
Probably they were not really married. If they were, either it was bigamous, or Frank divorced Annie Hall, married Addie Cornett, divorced Addie, and then remarried his first wife. But they were listed as husband and wife in the 1910 census.

1910 census of Kanawha precinct, Fayette Co., West Virginia, enumerated 3 May, lists on p. 17A as household #335, family #349, in the same house as Addie’s parents:
