The Ohio deaths index lists Mary Loraine Meeks, age 88, born 25 Feb 1906 in Rhode Island, died 10 Oct 1994 at an out of state hospital, father’s surname Fassett, widowed, 4 years college.
I don’t know where the Rhode Island comes from; perhaps Ky was misread as RI somehow.

Lorraine’s husband William is in the preceding household with his parents, making one wonder if this should have been recorded as a combined household.
1940 census of Boyd Co., Ky, Ashland ward 6, ED 10-32, enumerated 5 April, lists on p. 19B as family #60, at 941 45th St:
Meeks, William H., 43, salesman private school; Loraine, wife, 35; Richard, son, 11; Emily Ann, dau, 11; Callaway, son, 6; Mary M., dau, 4; Virginia J., dau, 2. All born in Kentucky, living in rural Bath Co., Ky in 1935.
