His WWI draft registration card, issued in 1917/1918 (the images on Ancestry do not show the side of the card that has the date it was filed), says Sam Slusher, age 21, residence Wagner, Ky, born 27 Dec 1896 in Rowan Co., father’s birthplace Elliott Co., Ky, employer Joe Slusher, nearest relative Joe Slusher, Wagner, Ky.

Most of my information on this family, such as birthdates, is from correspondence in 1993 with their daughter Mabel Slusher Forman of Clearfield, Ky.
Census
1920 census of Rowan Co., Wagoner precinct, lists on p.9 as family #73/76 (right after his parents):
Slusher, Samuel, 22; Lizzie, 22; Denton, son, 1/12. All born in Kentucky.
1930 census of Rowan Co., Ky, Wagoner precinct, taken 8 April, lists on p. 3B as family #45:
Slusher, Sam, 33, farmer, age at first marriage 22; Lizzie, wife, 33, age at first marriage 22; Geneva, dau, 9; Virginia, dau, 5; Fay, dau, 1 10/12; <illegible>, Leonard, cousin, 22. All born Ky/Ky/Ky.
I cannot figure out Leonard’s surname. It looks like Uurttet, or possibly Werttet, which doesn’t come close to the names of any known cousins of Sam or Lizzie. Lizzie’s maiden name was Pettit, but it sure does not appear to begin with a P.
1940 census of Rowan Co., Ky, Clearfield, enumerated April 30, lists on p. 11B as family #174, on Craney Creek:
Slusher, Sam, 43, farmer; Lizzie, wife, 43; Geneva, dau, 19; Virginia, dau, 16; Faye, dau, 11; Joe, son, 9; Mabel, dau, 4; Sam Jr., son, 4/12. All born in Kentucky, living in same house in 1935.
The preceding household is headed by Sam’s brother Willie, and the next one by his sister Rachel, with whom their widowed mother is living.
