His date of death is from his gravestone, a double one with Lenora, pictured on Findagrave.com.
The birth index at Kentucky Vital Statistics in Frankfort lists Lenora M Pettit, born 26 May 1912 in Breathitt Co., mother's maiden name Belle Brown. Her brother David H. Pettit says that she was really born in Clearfield. Due to a numerical coding error, most births in Rowan Co. in the periods 1911-1917 and 1940-1968 are indexed as being in Breathitt Co., even when the actual birth certificate says Rowan Co.

The SSDI lists Lenora M. Shackelford, born 26 May 1912, died 20 April 1999, last residence College Corner, Butler Co., Ohio, SSN issued in Ohio in 1967.
Her obituary reported “Lenora Mae Moore, 86, of College Corner, Ind., died Tuesday, April 20, in Riley, Ohio.
Born May 27, 1912, in Morgan County, Ky., she was the daughter of John and Isabelle (Brown) Pettitt. She was a member of Westgate Church of God in Oxford.
She was preceded in death by her first husband, Clarence Moore, in 1953; her second husband, Edward Shackelford, in 1978; a son, Charles Moore, in 1988; a daughter, June Helton, in 1998; four sisters; three brothers; and three grandchildren.
Survivors include daughters and sons-in-law, Margaret and Lewis Tomlin of Laurel, Ind., Phyllis and William Hardy of Cynthiana, Ky., Sylvia and Howard Weatherly of Eaton, Carol and Patrick Norris of Lawton, Okla; a son and daughter-in-law, Wilbur and Doris Moore of Huntington, W. Va.; 21 grandchildren; several great-grandchildren and great-great-grandchildren; nieces, nephews and cousins.
Funeral services were Friday, April 23, at Barnes Funeral Home-Eaton Chapel, with the Rev. Clarence Brannon officiating. Burial was in Concord Cemetery in Dixon Township, near Eaton.”
She is buried with her first husband Clarence Moore, and her gravestone has the birthdate 27 May 1912. That seems to be what she celebrated as her birthday, even though the birth certificate and Social Security say otherwise. Perhaps a birth near midnight that the family chose to place on the following day?
