Apparently the person filling out the form did not see the vertically aligned “Father | Mother” labels, and filled out the parent fields with Ida’s own married name and maiden name.

The second one records Ida Mert McKinzey, age 25, birthdate blank, born in Ky, died 29 Mar 1930 in Cogswell, Rowan Co., Ky, marital status blank, father Tom Ramey, mother’s maiden name Martha Riffe, both born in Ky, cause of death mitral regurgitation of heart, would not have a doctor, informant blank, burial in Salt Lick.
