The Kentucky birth index lists Nellie Madeline Alderman, born 15 Nov 1917, mother’s maiden name Stella M. Royse. It is a delayed certificate, filed in 1997.
There is also an entry for Nelley M. Aldersin, born 15 Nov 1917 in Breathitt Co. (a misindexing of Rowan Co.11), mother’s maiden name Stelley Rakey. (Either the second one was filied as a correction, since she ended up going by Madeline, or perhaps there were so many discrepancies that they couldn’t find the original.)
The SSDI lists Madeline Black, born 15 Nov 1917, died 18 Feb 2001, last residence Evansville, Vanderburgh Co., Indiana, SSN issued in Kentucky in 1963.
