His death certificate records John B. Cassity, age 29 years 5 months 6 days, born 17 Aug 1880 in Ky ( (age should be 39 years; the 1880 looks almost like 1890 and I think someone misread it and then computed the age from there) , died 11 Mar 1920 in Redwine, Morgan Co., Ky, father R. B. Cassity, mother’s maiden name Nancy Lemaster, cause of death typhoid fever, informant R. B. Cassity, Redwine.

Usually called Louettie, all one word. Several online family trees at Ancestry.com give her name as “Lonettie”, but that appears to be due to a mistranscription of a ‘u’ as an ‘n’, such as in their daughter Ethel’s record in the Ky birth index.
