Her death certificate records Josephine Johnson, age 79 years 9 months 20 days, birthdate blank, born in Kentucky, died 4 Sep 1946 in Salt Lick, Bath Co., Ky (however, the medical certification says that she was last seen by the physician on 5 Sep 1946 at 11 pm), widowed, father Richard Johnson, mother’s maiden name Viola Johnson, both born in Ky, informant Pink Johnson, Salt Lick, burial in Dickerson Cemetery.

Her son Pinkard has apparently mistaken the purpose of the parental fields and entered his own parents rather than his mother’s.
