His death certificate lists George Bryon Valk, age 59 years 10 months 12 days, born 22 Oct 1871 in Walkerton, Indiana, died 4 Sep 1931 in Shelby, Richland Co., Ohio, married, husband of Eliza Jones Valk, occupation sulphate maker at tube mill, father James Valk, mother’s maiden name Margaret Cox, both born in Pennsylvania, cause of death acute lobar pneumonia, informant Frank Valk, Shelby, Ohio.
Note that this is the same street address that George and Eliza were recorded at in the 1930 census, with his age given at that time as 69.

The death date of 1945 is from her gravestone. I can't find her in the death records for either Kentucky or Ohio. Should look for an obituary.
