The Kentucky birth index lists Clarence Wells, born 7 Jan 1913 in Menifee Co., mother’s maiden name Ollie Wells.
His death certificate records Clarence Wells, age 3 days, born 7 Jan 1912 (sic) in Kentucky, died 10 Jan 1913 in Wellington, Menifee Co., Ky, occupation house maid, father fields blank, mother’s maiden name Ollie Mary Wells, born in Kentucky, cause of death congenital deformity, informant R. C. Wells, Wellington, burial in J. H. Wells.
(The clerk apparently had not become accustomed to entering the new year’s date yet.) Ollie’s middle name might be May instead of Mary - it looks like Ma followed by a w with a tail, or iy with no dot on the i. So it could be a squished r, or just getting exuberant with the loops on a y.
