His death certificate records Willie Sherman Perry, age 59 years 5 months 29 days, born 9 July 1883 in Morgan Co., Ky, farmer, married, husband of Lizzie Perry, father Anderson Williams, mother’s maiden name Sarah E. Jones, cause of death heart attack, informant Emmett Perry, Ashland, Ky, burial in Blaze, Ky.
The handwriting on this page looks like it was originally entered in pencil or very light ink, and then overwritten in dark ink - virtually every letter has a ghost behind it. So I think what happened with the “Anderson Williams” is that it originally said “Anderson William” with no surname, intending “Anderson William Perry”, with the informant assuming that a father’s surname could be assumed for male decedent, and then whoever went over it to darken the text added the s.

Two households below is the family of Leroy C. Lewis and Julia Jackson. Their widowed daughter Lizzie, Willie’s future second wife, is living with them with her two children Alline and Augustus.
