His death certificate record Oliver Henry Perry, age 49, born 1848, died 18 May 1918 in Buffalo precinct, Carter Co., Ky, married, farmer, father Wm N. Perry, mother’s maiden name blank, both born in Kentucky, cause of death accidental (crushed by log), informant Ralph Bloomfield, Rooney, Ky, burial in McGlone Cemetery.
His obituary reported:
“PAT PERRY KILLED
Pat Perry, a prominent citizen of Rooney, this county, while hauling logs Saturday with a team of horses was killed. It seems that the team became frightened and ran and that the log which he was hauling struck him in the breast, killing him almost instantly. Dr. J. M. Rose , of this place, was summoned but he died before he reached him.”
There are two FindaGrave memorials for him. One has the name Oliver Henry Perry, and the other gives his name as Oliver Hazard “Pat” Perry.
