The SSDI index lists Millard Mayse, born 10 July 1893, died 3 May 1995, last residence Elliottville, Rowan Co., Ky.
The Kentucky death index lists Millard Mayse, age 100, died 3 May 1895 in Morehead, Rowan Co., Ky.
His gravestone in Forest Lawn has his birth date as 15 July 1894. Considering the three variations here, he probably did not know his real birth date. I cannot find his parents in the 1900 census; in April 1910 he was listed as age 16.
There was another Millard Mays, born 28 Dec 1896 in Elliott Co, son of Carter Mays and Lucinda Burton, who lived most of his life in Carter Co., and died 2 March 1970 in Ashland.
