Pergram, mother’s maiden name Nannie Botts, both born in Ky, cause of death broncho pneumonia, informant Frank Moore, Sudith, Ky, burial in Pergram graveyard. (The medical statement on this certificate is signed by Dr. Reese B. Kash, and the registrar’s signature is his wife Bethel.)

The reported birth date is apparently due to mistakenly entering the day and month of death in both fields. The 1900 census records her birth date as Aug 1855, and her gravestone has it as 16 Aug 1852.
