His death certificate records Samuel H. Collins, age 51 years 6 months, born May 1867 in Ky, died 4 Dec 1918 in Rowan Co., Ky, occupation fireman on R.R., married, father C. W. Collins, mother’s maiden name Mary Collins, both born in Ky, cause of death influenza, informant Cyet (?) Perry, Morehead, Ky.
There is no gravestone for him in Rowan Co., but there is a gravestone in Sargent Cemetery in Morgan Co. (where his father and his brothers William and Hiram are buried) with the inscription S. H. Collins, Born July 28, 1866, died Dec. 13, 1918. There is no death certificate for any Collins who died 13 Dec 1918. So I suspect this is his grave, and that when the stone was inscribed, they got his death date wrong by 9 days. Unfortunately there is no guarantee that the birthdate on the stone is correct either.

Laime, father Thomas Johnson, mother’s maiden name Mary Johnson, both born in Ky, cause of death cerebral hemorrhage due to chronic myocarditis, informant Mrs. Frank Maxey, Morehead, Ky, burial in Caudill Cemetery.
