Thomas was adopted, and apparently went back to his birth surname after his marriage, which is somewhat unusual.
I am not sure that the S. in Thomas S. Coffey (from the 1910 census) stands for Smithly. It might be a real middle name.

Rowan Co. marriage book 6, p. 595, records the marriage on 27 May 1909 of Tommie Smithly, 34, born Greenup Co., son of Jake Smithly b. Pa and Bettie Smithly b. Magoffin Co., to Laura Edna Sorrel, 18, daughter of M. C. Sorrell and Anna Sorrell. Marriage performed at bride’s residence by H. G. Alderson, witnesses Willie Flanery and Doc Sorrell.
This is the only record I have seen that gives her name as Laura. Every other record says Louie.
