The Kentucky birth index lists Edna M. Shelton, born 6 Oct 1918 in Rowan Co., mother’s maiden name Ethel Markwell.
She must also be the Verna Shelton whose death certificate records that she was born 5 Oct 1918 in Ky, and died at age 3 days on 8 Oct 1918 in Morehead, Rowan Co., Ky, father Linsey Shelton, mother’s maiden name Ethyl Markwell, both born in Ky, cause of death unknown, informant blank, burial in Baldridge Cemetery, Clearfield, Ky. (It must be an unnmarked grave, because there are no Shelton stones in that graveyard.)
Pearl is the only child in the 1920 census, and there is no death certificate for any other Shelton child in 1918 to 1920, nor any birth record for a Verna or similar name. Hence twins are unlikely. Either Linzy and Ethel changed their minds about what to name her after the birth was recorded, or else a middle initial of V was mistranscribed as M.
