McClurg, Farmers, Ky, burial in Siloam Cemetery.

The birthdate is very questionable, see the census notes. Her gravestone has no dates.

She was listed in the 1900 census3 as age 14, born May 1886, in the 1910 census as age 19, in the 1920 census as age 24, and in the 1930 census as age 36. The 1910 one is probably the closest to accurate. (The ages in that census for her brothers Andrew Marion, Roy and Luther match best with the dates given in their draft registrations.)

1930 census of Rowan Co., Ky, Pine Grove precinct, enumerated 18 April, lists on p. 9A as family #142/161:
Collins, Della, 36, head, widowed, age at first marriage 32, b. Ky Ky Ky, no occupation; Jaunita, 4, dau, b. W Va Ohio Ky.

She is sharing a dwelling number with her father, but they have separate family numbers.
