He was sometimes called John Perry Lewis Jr., to distinguish him from his first cousin of the same name, who was about 5 years older.

Morgan Co., Ky death records list Margaret Dyer, age 52, died 2 Aug 1874, single, born in Ky, parents John & Mary Dyer, cause of death palsy.

I am not sure that all of Margaret Dyer’s children were by Jack Lewis, but he is so recorded on the death certificates of all of her children that I’ve located, so I will assume that is the case. Since Jack remained married to Clarinda McClure the entire time that he was fathering a passle of children by Peggy Dyer, it appears that he essentially had two simultaneous families, a formal one and one on the side.
