The Ohio deaths database lists Sarah E. Boorman, age 86, died 27 July 1966 at Southern Ohio Medical Centery, Portsmouth, Scioto Co., Ohio, widowed.
The death record of her daughter Nellie gives her maiden name as Roseberry, and the death record of her son Grover gives her maiden name as Sexten. There is a good match for a Sarah E. Roseberry, but no match that I can find for a Sarah Sexton.
