His death certificate records John Hay, age 66, birthdate blank, born in Elliott Co., Ky, died 14 Dec 1924 in Dry Fork, Lawrence Co., Ky, father Wm Hayes, mother’s maiden name Polly Burton, both born in Va, cause of death cerebral apoplexy, informant Grimsley Hays, New Boston, Ohio. (The certificate really does contain the three spellings, Hay/Hayes/Hay.)
Be careful not to confuse him with John C. M. Hay born 28 April 1857 in Morgan Co., son of Henry Hay and Eunice “Nicy” Robinson.

All children born Ky Ky Ky.

At the top of the page it says that lines 13 through 27 inclusive are the County Poor House, but it appears that the “poorhouse” consists of only 5 children and one adult being boarded at a private farm. In modern parlance we would consider this to be a family that participated in fostering wards of the county.
