I cannot find a death certificate or cemetery record for her. She presumably died before 1920, when her parents were caring for her children.

1920 census of Summit Co., Ohio, Akron ward 4, enumerated 13 Jan, lists on p. 9B as family #188/218, at 791 Noah Ave:
Lyon, Clint C., 28, rubber worker at rubber factory; Elena A., dau, 3; Walter G., son, 1 2/12, b. Ohio Ky Ky; Lyon, Adelia, mother, 48; Zettie C., father, 62. Patrick, Carman A., niece, 5; Herman, nephew, 2. All except the baby Walter born Ky Ky Ky.
Based on the Kentucky birth index, the mother of the two Patrick children was Zettie and Della’s daughter Ida Ellen. I do not know the given name of her husband. The relationship for “Herman” should be niece, not nephew. Probably the census taker misheard “Herma Ann” as Herman, and assumed the gender on that basis.
