His obituary as printed in the Morehead News spelled his name as Forman:
“Goldman G. Forman, 78, of Lower Licking Road, Morehead, died Sunday, Dec. 6 [1987], at St. Claire Medical Center. A native of Rowan County, he was the son of the late Ballard and Polly Ann Lewis Forman. His wife, Gladys Hall Forman, died in 1986. Mr. Forman was a member of the Church of God and a retired employee of Lee Clay Products Company of Clearfield where he worked for 46 years. Survivors include two sons, Doug Forman, Morehead, and Kenneth C. Forman, Spring Hill, Fla., a daughter, Dorothy Armstrong, Alexandria, Ind. Also, a brother, Paul Forman, Morehead; a sister, Zora Crosthwaite, Ogden, Utah; seven grandchildren and nine great-grandchildren. A son, Larry Forman, preceded his parents in death in 1964. Services were conducted Wednesday, Dec. 9, at Northcutt and Son Home for Funerals Memorial Chapel with the Rev. Mitchell W. Burch officiating. Burial was in Forest Lawn Memorial Gardens. Pallbearers were Walter Rybka, David Forman, Denver Forman, Chalmer Utterback, Earl Blair and Walter Forman.”
