The Kentucky death index lists Ballard Forman, age 89, died 21 May 1961 in Rowan Co., certificate Vol. 54, #26773.

Her death certificate (Vol. 31, #15449) records Mary Anne Forman, age 66, age 66 years 3 months 4 days, born 3 Apr 1881 in Morgan Co., Ky, died 7 July 1948 in Rowan Co., married, wife of Ballard Forman age 75, father Daniel Caudill, mother’s maiden name Lewis, both born in Ky, cause of death stroke, informant Ballard Foreman, Rt. 3, Morehead, Ky.

I cannot find a Daniel Caudill married to a Lewis. So I do not know if Caudill was her maiden name and she was previously married to a Lewis before marrying Ballard Forman, or if she was born out of wedlock and carried her mother’s surname. Considering the number of other records that list her maiden name as Lewis, most likely the latter. (Yet another time when one really wishes the 1890 census had survived.)

Morgan Co. marriage records list marriage of Ballard Forman and Polly A. Lewis on 17 May 1897 at J. M. Maxey’s by O. J. McKenzie, witnesses John Martin and John Utterback.

Forman, Ballard T., 37; Mary A., wife, 30; married 15 years, 6 children, 6 living; Audie, son, 12; Olah, dau, 10; Kelly, son, 8; Tom, son, 6; Ollie, son, 3; Goldman, son, 7/12. All born in Kentucky.

1920 census of Rowan Co., Ky, Pearcy precinct, p. 212B, lists as family #37/38:
Foreman, Ballard, 46; Mary A., wife, 38; Oddie, son, 21; Kelly, son, 18; Tommie, son, 15; Ollie, son, 13; Goldman, son, 10; Zora, dau, 7; Paul, son, 10/12. All born in Ky.

1930 census of Rowan Co., Ky, Pearcy precinct, enumerated 7 April, lists on p. 2A as family #28:
Foreman, Ballard, 56, age at first marriage 25, farmer; Pollie, wife, 50, age at first marriage 18; Zora P., dau, 16; Paul, son, 11; Mattie, dau, 5; Kathleen, dau, 3. All born Ky Ky Ky.

The preceding two households are headed by Clester Foreman age 22 with wife Hallie age 20, and Wess Foreman age 60 with wife Jennie age 59.

I cannot find Mattie and Kathleen in the Kentucky birth records, and considering Polly’s age, they may be granddaughters instead of daughters.
