His death certificate records Clate Morris, age 21 years 8 months, born in Rowan Co., Ky, birthdate blank, died 20 Jan 1933 in McKinzie precinct, Rowan Co., Ky, farmer, married, father Clate Morris, mother fields blank, cause of death “spinal trouble and pneumonia this is his fathers statement as there was no physician”, informant name blank, informant’s address Bertis, Ky. The physician statement area is signed Cleve Morris, Paragon, Ky. (It appears that some of the date was entered in the wrong spaces.)

Hope by R. A. Alfrey, minister of the Baptist church, witnesses Roy Alfrey and Sadie Johnson.
