The database of applications for military veteran headstones contains an application dated 17 May 1939, filed by James Nichols, Dunkirk, Indiana, for a stone for Arthur L. Nichols, died 6 Dec 1938, enlisted 7 May 1918, honorable discharge on 11 July 1919, private in Co. D, 22nd Engineers, buried in I.O.O.F. Cemetery, Dunkirk, Indiana. (The form does not include a space for date of birth even though it is placed on the markers; that is apparently obtained from military records.)
